1. The LSC-Tomball Nursing Program is not its own college or school. You must apply to the admission office in the Commons area (or be a student at a Lone Star College System campus) before you can apply for the nursing program. Submit transcripts and placement test scores from all non-Lone Star College System colleges and universities to the transcript department. Allow time for the transcripts to be received, analyzed and entered into the computer system by the Admissions department. All courses that you wish to transfer in and apply to the Nursing program application must be entered into the computer system prior to turning in the Nursing application. If you have completed courses at another college within the Lone Star College System (Kingwood, North Harris, Montgomery, Cy-Fair) you do not need to submit transcripts because we all share one computer system. Transcripts must be analyzed and posted in the computer in order to submit your online application for the nursing program.

2. You need to be TSI/SSI complete to apply. If you are unsure of your status, ask in Admissions/Enrollment Services.

3. Change your major in Admissions to Basic Track program (AAS.AADN), catalog year 2008. This step is required in order to submit your online application for the nursing program. Any changes made in Admissions or on any computer will take at least three business days to register on the online nursing application system.

4. Take the Nursing Pre-Entrance Exam (HESI Assessment Test, A2). This is required of all Nursing applicants. No other test scores can be substituted. If you take this test at Lone Star College-Tomball, your scores will show on your Advising Profile within 4-5 days (they are manually entered). You will need to have test scores before you can submit a completed application. You may use existing HESI scores as long as you have completed the Reading and Math sections. You can use old HESI scores (as long as you meet the minimum passing scores) as they do not expire. The test is $25 and can be paid by cash, check, and credit or debit card in the TC Business Office. Print an Authorization To Test from the Nursing website and take it with you to the Business Office.  Additional instruction can be found on the actual Authorization To Test. You can pay for the test and return at a later date to take the exam but the Assessment Center does not make appointment, testing is first-come, first serve.

The HESI test is computerized and is given in the Assessment Center, Library building, room L-201. A practice book for the HESI A2 pre-entrance exam can be purchased on the HESI website, www.hesitest.com. A copy of this book is also on reserve in the TC Library.

5. The online application for the BASIC TRACK will be available on the website on Wednesday, January 14, 2009. Please read all instructions carefully. You will need to print a copy of BOTH the application and the score sheet and turn them in to the front counter of the Admissions Department by 4:00pm March 2, 2009. You must sign the application and supplementary documents confirming the information is correct, so, please check your forms for accuracy before you turn them in. All necessary required transcripts must be received and loaded into the computer by the Transcripts Office for at least two business days in order for the online nursing application to work properly. Please allow sufficient time so you can submit your application by the deadline. Any changes made in admissions or on any computer will take at least two business days to register on the online nursing application system.

Notification letters will be mailed in April. No information will be given over the phone. The program will begin in July/August.

6. No proof of immunization are due at the time of application, however, you are encouraged to begin the Hepatitis B vaccination series in January as this series takes six (6) months to complete. You will need the have the series completed by June (Bridge students) or July (Basic Track students). The Hepatitis B shot is available at NHC during scheduled Hepatitis B vaccination clinics. Please do not attach immunization records to your application.

If you need to receive a new tetanus shot because your previous tetanus shot is more than 2 years old, please request the Tdap (combined Tetanus, Diphtheria and Pertussis) vaccine rather than the older Tetanus/Diphtheria (Td) shot. If you received the Td or Tdap vaccination within the last 2 years, you will not need a new shot. All other immunizations required for new students will be outlined at orientation once you have been admitted into the program.

7. After you have been accepted into our program, you will be required to submit a Background Check from one our three approved vendors (information will be given out at new student orientation). You will also be required to pay for and submit to a urine drug screen. If you fail the required drug screen/criminal background check, you will not be allowed to enter in the program.

8. If you were dismissed from or failed a previous nursing program (Vocational Nursing, Associate Degree Nursing, Bachelor of Science in Nursing, etc.), you must submit both a letter from the Nursing Dean/Director of your previous program and a personal letter from you indicating your intent to apply for the Nursing program. Your letter should include a summary of your academic history, explain your withdrawal/failure, and outline how you will be successful in our program. These letters should be submitted in advance of the application deadline to allow sufficient time for review by the Nursing Director.
